Change in Income Continuation Insurance Claim Process Dates

The Department of Employe Trust Funds will continue to accept Income Continuation Insurance (ICI) claim forms using the current process through May 2000. Employer Bulletin Vol. 17, No. 4 (February 16, 2000), provided information on the new ICI claims process and advised that old claim forms (ET-5301 or ET-5310) should be destroyed and only new forms would be accepted effective April 1, 2000.

The implementation deadline for the new ICI claim process has been extended until June 1, 2000 to give you enough time to receive the new ICI claim packets and begin using them. Once you receive your supply of new ICI claim packets, please destroy the old claim forms (ET-5301 or ET-5310). Beginning June 1, 2000 we will only accept new claim forms.
