Correction to the Group Life Insurance Administration Manual

The Group Life Insurance Administration Manual (ET-1117), revised November 2001 contains an error in the State employee supplemental premium amount for age 65-69. The Employee Premium Rate - Chart A on page two of Chapter 9 incorrectly shows $.55 as the State employee supplemental premium amount.

The correct State employee supplemental rate for age 65-69 is $.59. Insert the enclosed corrected page into your Manual. We have also corrected the Group Life Insurance Administration Manual (ET-1117) located under "Employer" on our Internet site, etf.wi.gov.
