Attached to this Bulletin is an updated 2002-2003 Plan
Address/Phone/Fax Number List for employers to use to send Health
Insurance Applications to the Plans. Please use these addresses
rather than those listed on the inside back cover of the 2003 It's
Your Choice booklet.

Corrections/Additions to 2003 It's Your Choice
Booklets
Please note the following changes in the map and rate pages in
the It's Your Choice booklet for State of Wisconsin
employees, graduate assistants, and annuitants.
- Valley Health Plan appears as a non-qualified plan in the maps
in Barron, Dunn and Eau Claire counties and should appear as qualified.
- Valley Health Plan appears as a non-qualified plan in the rate
tables for Barron and Dunn counties and should appear as qualified.
- Please note the following change on the map page in the It's
Your Choice booklet for Wisconsin Public Employees (local
employers).
- Valley Health Plan appears as a non-qualified plan on the map
in Barron and Dunn counties and should appear as a qualified plan.
The above changes do not affect the employee/employer share in
any county and the rate tables and maps that appear on the ETF web
site are correct.
- Please note that Compcare North is a non-qualified plan in Waupaca
County.

- The 2003 It's Your Choice booklets for State of
Wisconsin Employees and University of Wisconsin Graduate Assistants
erroneously include Plan Description Pages for Medicare Plus $100,000;
this plan is not available to active employees.
- Please note: the Valley Health Plan benefit description page,
dental coverage section, erroneously states benefits are available
only for children 11 and under. The correct dental benefit is
one exam (exam, prophylaxis/cleaning, bitewing X-rays) per calendar
year for all members and for children 14 years and under, coverage
for fluoride treatment and sealants. (Page G-43 in the State of
Wisconsin Employees booklet, the Annuitants/Continuants booklet,
and the University of Wisconsin Graduate Assistants booklet; page
G-41 in the Local Government Booklet.)
