Dual-Choice Enrollment Period for Plan Year 2004
The Group Insurance Board will review the Dual-Choice enrollment
period at the June 3, 2003 meeting. Employee Trust Funds (ETF) will
notify employers when the Dual-Choice dates are set. Subsequent
Employer Bulletins will contain further detailed information and
instructions. During Dual-Choice enrollment, currently insured employees
may switch health plans and/or switch from single to family coverage
without restrictions. All coverage changes will be effective January
1, 2004.
Attend the Dual-Choice Kick-Off Meeting: October 1,
2003
Each year ETF hosts a Dual-Choice "Kick-Off" meeting
for state and local employers, payroll and personnel representatives,
health plan representatives, and ETF staff to share information
and discuss procedures regarding the Dual-Choice Enrollment period.
The 2004 Dual-Choice Kick-Off meeting has been scheduled as follows:

Attached is a map showing
directions to the Conference Center Building in Madison. Mendota
Mental Health Institute is located at the north edge of Madison
on Lake Mendota. From either the north or south take Interstate
90/94 to Highway 30 into Madison. From Highway 30 exit to Highway
113 north toward the Dane County airport. Stay on Highway 113 (Northport
Drive) to Troy Drive or Knutson Drive (Look for the tall smoke stack).
Parking is available in several lots located at the facility:
- Behind the Engineering Power Plant next to Building 107. Follow
the service road from the parking lot to Lorenz Road. The entrance
to the Conference Center is on the south side of the building.
- On Main Drive between the Administration Building and Lorenz
Hall.
- On Memorial Drive in front of Goodland Hall.
Arrange Dual Health Fairs Early
Representatives from group health insurance plans will be available
to provide information and answer questions at employer health fairs.
Local employers and state agencies are responsible for arranging
their own health fairs. Please allow plenty of time for It's
Your Choice books to arrive before you schedule your fairs
or meetings with staff.

Attached to this Bulletin is an updated copy of the Health Plan Contacts sheet (ET-1728) revised
05/2003. This list contains each Plan's contacts for membership
and coverage, claims, supplies, employee assistance program, and
complaints/grievances.
