Bulletin Announces Dual-Choice Enrollment Period
and Kick-Off Meeting Dates
Mark Your Calendars: Dual-Choice Enrollment for Plan
We take this opportunity to announce the dates associated with
the Dual-Choice Enrollment period for Group Health Insurance coverage
in 2005. Watch for subsequent Employer Bulletins containing
more detailed information and instructions.
Dual-Choice Enrollment Period Set for October 11-29,
State agencies and local employers who participate in the Group
Health Insurance program should mark their calendars. The Group
Insurance Board has established October 11-29, 2004 as the Dual-Choice
Enrollment period for 2005. During this period, currently
insured employees may switch from one health plan to another
plan and/or from single to family coverage without restrictions.
All coverage changes will be effective January 1, 2005.
Attend the Dual-Choice Kick-Off Meeting: September
Each year the Department of Employee Trust Funds (ETF) hosts a
Dual-Choice “Kick-Off” meeting for state and local employers,
payroll and personnel representatives, and health plan and pharmacy
benefit representatives. ETF staff share information and discuss
procedures regarding the Dual-Choice Enrollment period. The 2005
Dual-Choice “Kick-Off” meeting is scheduled as follows:

Arrange Dual-Choice Health Fairs Early
Representatives from group health insurance plans and Navitus Health
Solutions personnel will provide information and answer questions
at employer health fairs. Local employers and state agencies are
responsible for arranging their own health fairs. Please allow plenty
of time for It’s Your Choice books to arrive before
you schedule your fairs or meetings with staff.

Updated Plan Contact List
Attached to this Bulletin is an updated copy of the Health
Plan Contacts sheet (ET-1728) revised 05/2004. This list contains
each Plan’s contacts for membership and coverage, claims,
supplies, employee assistance program, and complaints/grievances.
