Important Reminder: Revisions to Continuation-Conversion
Notice (ET-2311)
In June 2005, the Department of Employee Trust Funds (ETF) provided
an Employer Bulletin* outlining the U.S. Department of Labor’s
(DOL) final rules setting minimum standards for the timing and content
of the notices required under the continuation coverage provisions
of the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA
see Employer Bulletin, Vol. 22, No. 10, June 21, 2005.)
ETF recently revised the Continuation - Conversion Notice (ET-2311)
to comply with the COBRA Final Rules and included a copy with Employer
Bulletin, Vol. 22, No. 10. We remind you that in order to comply
with the new regulations, the revised Continuation - Conversion
Notice must be used in lieu of any notice with prior revision dates.
For your reference, included is an additional copy and we ask that
you please discard any Continuation - Conversion Notices
with a revision date prior to 04/2005.
NOTE: Beginning November 1, 2005, ETF will reject any Continuation
- Conversion Notice received with a revision date prior to 04/2005.
