Local Health Insurance Administration Manual Revised
This bulletin accompanies your copy of the revised Local
Health Insurance Administration Manual (ET-1144, rev. 07/2005).
Materials and subchapters have been renumbered and new chapters
and subchapters added. The manual contains revisions and clarifications
previously published in Employer Bulletins as well as clarifications
suggested by Department of Employee Trust Funds (ETF) staff and
Wisconsin Retirement System employers participating in the Wisconsin
Public Employers Health Insurance program.
The Local Health Insurance Administration Manual includes updated
versions of the Group Health Insurance Application (ET-2301) and
Health Insurance Information Change form (ET-2329), which will be
available for distribution in the near future. ETF anticipates both
forms will be printed within the next four weeks and will notify
you, via an upcoming Employer Bulletin, of their availability and
explain substantive changes to the forms.
NOTE: Additional revisions were made to the Group Health Insurance
Application and the Health Insurance Information Change form after
printing the revised manual. Please insert the attached pages, reflecting
the updated forms, their corresponding instructions, and other changes
into the accompanying copy of the Local Health Insurance Administration
Manual. The revised pages are:
- Chapter 3, pages 5/6, 7/8, 9/10
- Chapter 4, pages 7/8
- Chapter 5, pages 19/20, 21/22, 24/24
- Chapter 6, pages 9/10, 11/12, 13/14
We encourage you to visit our Internet site, http://etf.wi.gov,
to view and print any or all chapters of the Local Health Insurance
Administration Manual. Additional paper copies of the manual may
be ordered by following the instructions found in Subchapter 109
of the manual.
Please consult the Local Health Insurance Administration Manual
as a first-step resource when program questions or concerns arise.
