Unity Insurance ID Cards Mailed Early

Late the week of November 21, Unity Health Insurance mailed ID cards to all of its current subscribers for coverage effective January 1, 2017. This may cause confusion because It's Your Choice open enrollment elections are still being processed.
Please note:

  • IYC applications can be entered by employers into myETF Benefits until December 9, 2016. (There is a separate process for late applications.)
  • Changes that Unity subscribers made that were not entered yet, or were entered recently, will not be reflected on the new Unity ID card, such as:
    • Adding or dropping a dependent
    • Changing coverage (single to family, family to single)
    • Changing a Primary Care Provider (PCP)
    • Terminating coverage with Unity (opting out of coverage or switching to a different health plan)

Unity will resend new ID cards if they receive the following enrollment changes:

  • Adding or dropping a dependent
  • Changing coverage
  • Changing a PCP

Subscribers who terminated coverage with Unity during IYC open enrollment will receive an ID card from their new health plan, if one was selected. If the subscriber opted out of coverage for 2017, ETF will evaluate whether a special mailing to these individuals is needed.
