Opt Out of Uniform Dental Benefits

Beginning January 1, 2016, you can choose your health insurance, with or without Uniform Dental Benefits, if your employer offers the Uniform Dental Benefit. Ask your benefits office about available options.

If you are currently enrolled in a health plan, you will be automatically enrolled in Uniform Dental Benefits and can choose to opt out during the It’s Your Choice open enrollment period. New employees may choose health insurance, with or without dental, if offered by their employer, during the application process.

Opt out of Uniform Dental Benefits by filing a Group Health Insurance Application/Change Form (ET-2301). You can make a change in myETF Benefits, or complete a paper application and submit it to your benefits office if you are an employee, or to ETF if you are a retiree. If you do not opt out during It’s Your Choice open enrollment (October 5-30) you will have Uniform Dental Benefits in 2016, if offered by your employer. Check your premium rates for the cost difference between medical benefits with and without Uniform Dental Benefits.



Enroll or Make Changes:
myETF Benefits provides employees the ability to submit changes to their current coverage including address/name changes, new enrollments, and qualifying event changes to their existing coverage online.
