Opt Out of Medical Benefits

Beginning January 1, 2016, state employees may be eligible to receive up to $2,000 from your employer if you opt out (waive coverage) of state group health insurance coverage. To be eligible you cannot have opted out in 2015, and you also cannot be covered under the State of Wisconsin Group Health Insurance Program as a dependent. You are not eligible for the opt-out incentive in 2017 if you are covered by the State of Wisconsin Group Health Insurance Program even one day in 2017 (see FAQs for more information).

You are required to submit a paper Group Health Insurance Application/Change (ET-2301) form to your payroll office during It’s Your Choice open enrollment to receive the opt-out incentive for 2017 (be sure to complete Sections 1, 14 and 15 of the paper application). New hires must also submit a Group Health Insurance Application/Change (ET-2301) form within 30 days of your hire date indicating you are opting out of health insurance.

UW Graduate Assistants and Crafts Workers are not eligible for the opt-out incentive.

Note: Due to November 11 being a federal holiday, mailed applications must be postmarked by November 10, 2016. ETF offices will be open to accept applications.
