Care Coordination Rating
The “Care Coordination Ratings” in the table below looks at how well a plan does managing the various health care services so they work effectively for you.
The types of measures that make up the rating include:
- Managing care over time
For example: Managing medications over time related to antidepressants, asthma and children on attention-deficit/hyperactivity disorder (ADHD) medication.
- Managing “hand-offs”
“Hand-offs” mean things like transferring from the hospital to back home. Coordinating care from one setting to another is important to keep bad things from happening. An example of one of the measures here is readmissions that happen shortly after leaving the hospital.
- What people say
This includes what people say about how well their plan is doing in helping with coordinating their care.
