Open Enrollment October 2 – 27, 2017

If you are currently enrolled in Gundersen Health Plan, Physicians Plus or Unity (Community or UW) and do not wish to change health plans during open enrollment you will automatically be enrolled in either Quartz – Community or Quartz – UW Health.

You do not need to take action during open enrollment unless you would like to select a health plan other than Quartz for 2018. If you take no action during open enrollment you will be automatically enrolled in Quartz – Community or Quartz – UW Health based on your current primary care physician’s network.

What’s New for 2018

We are pleased to announce Gundersen Health Plan, Physicians Plus Insurance Corporation and Unity Health Insurance have joined forces under the new brand name of Quartz – a new name in health insurance products and services. Quartz – Community network offers access to doctors and hospitals throughout southern Wisconsin, including Gundersen Health System and guaranteed access to the specialists at UnityPoint Health – Meriter Hospital and Clinics, UW Hospital and Clinics and American Family Children’s Hospital.

Provider Directory

Go to ChooseQuartz.com to access a provider directory. Select “Find a Doctor for State Employees.” Select the “Community Network” link. To request a printed provider directory be mailed to you, call 1-844-644-3455.

Referrals, Prior Authorizations, Out-of-Network Care

Referrals are not needed when receiving care from in-network providers. Prior authorization is required for certain services, procedures and for out-of-network providers before you receive care. You will receive written notification upon approval or denial for the prior authorization. You may check the status of your request at QuartzMyChart.com. If you are unsure if a service, test or procedure requires prior authorization or a referral, call 1-844-644-3455 prior to the appointment or visit QuartzBenefits.com/priorauth.

Service Area

Adams, Columbia, Crawford, Dodge, Eau Claire, Fond du Lac, Grant, Green, Iowa, Jackson, Jefferson, Juneau, LaCrosse, Lafayette, Marquette, Monroe, Richland, Rock, Sauk, Trempealeau, Vernon, Walworth and Waukesha counties.
Limited provider availability in: Chippewa, Clark and Wood counties as well as providers located in Cambridge and Mazomanie.

Emergency and Urgent Care Outside Service Area

If you require emergency or urgent care when outside of the network, seek care at the nearest medical facility. Contact Quartz Customer Service at 1-844-644-3455 or your primary care physician if assistance is needed to determine the appropriate level of care. Follow-up care must be received from an in-network provider unless approved by Quartz.

Mental, Behavioral Health and Substance Abuse Services

For assistance in accessing mental, behavioral health or substance abuse services, call UW Health - Behavioral Health Care Management at 1-800-683-2300.

Contact your primary care provider for after-hour or out-of-area care instructions.
