Accidental Death and Dismemberment (AD&D)
(State Employees Only)

Underwritten by The Hartford

A free-standing accidental death and dismemberment policy is offered to state employees. There is no employer contribution. You pay the full premium, but you may pay through payroll deduction.

Accidental death and dismemberment coverage can help to provide 24-hour, worldwide insurance protection in the event of a severe injury, paralysis or death resulting from an accident. Benefits are payable for losses which occur within 365 days of the accident. For a few dollars per month, you can buy substantial accidental death and dismemberment coverage. Because the plan offers individual (or family) policies at group rates, you get the protection of a major insurer, the Hartford, without the high cost.

If you are a Wisconsin state employee in an active, full-time, part-time (at least 20 hours per week) or seasonal status, you are eligible for this coverage. You cannot be refused acceptance, and no evidence of insurability is needed.

Please see The Hartford's brochure for more information.

For questions related to enrollment or billing, see the payroll and benefits representative at your workplace. For further questions about billing your payroll representative may call (800) 523-2233. For claims questions, call (800) 563-1124.
