Description

Your life is non-stop. We think your health insurance should be able to keep up. Our programs, tools and resources are built to keep you healthy, happy and on-the-go. Because when you’re feeling good, life is good.

From giving you access to the doctors you want to see, to providing exceptional customer service, every time – everything we do is because of you. That’s the Quartz difference.

Quartz networks are designed to help you get the care you need, when and where you need it. Quartz - Community Network provides you with a broad selection of providers, hospitals and clinics located outside of Dane County and the communities of Cambridge, Marshall and Mazomanie.

We take pride in providing each community with exceptional service and access to the area’s top-rated health care. Visit ChooseQuartz.com for more information.

What’s New for 2020

And, there’s no cost to you, because Quartz has your back. To learn more about the program and find out if you’re eligible, visit QuartzBenefits.com/MobileBack.

Referrals, Authorizations, Out-of-Network

Referrals are not needed when receiving care from in-network providers. Prior authorization is required for certain services, procedures and for out-of-network providers before you receive care. You will receive written notification upon approval or denial for the prior authorization. You may check the status of your request at QuartzMyChart.com. If you are unsure if a service, test or procedure requires prior authorization or a referral, call 1-844-644-3455 prior to the appointment or visit QuartzBenefits.com/priorauth.

Emergency and Urgent Care

If you require emergency or urgent care when outside of the network, seek care at the nearest medical facility. Contact Quartz Customer Service at 1-844-644-3455 or your primary care provider if assistance is needed to determine the appropriate level of care. Follow-up care must be received from an in-network provider unless approved by Quartz.
