Quartz MobileBack is brand-new, app-based program designed to help manage low back pain. Through a mobile app on your smartphone or tablet, you’ll get easy-to-follow exercise routines designed by spine physical therapists for your specific type of low back pain. And, there’s no cost to you, because Quartz has your back. To learn more about the program and find out if you’re eligible, visit QuartzBenefits.com/MobileBack.
Go to ChooseQuartz.com to access a provider directory. Select “Find a Doctor for State Employees” and select the “Quartz - UW Health” link. To request a printed provider directory be mailed to you, call 1-844-644-3455.
Referrals, Authorizations, Out-of-Network
Referrals are not needed when receiving care from in-network providers. Prior authorization is required for certain services, procedures and for out-of-network providers before you receive care. You will receive written notification upon approval or denial for the prior authorization. You may check the status of your request at QuartzMyChart.com. If you are unsure if a service, test or procedure requires prior authorization or a referral, call 1-844-644-3455 prior to the appointment or visit QuartzBenefits.com/priorauth.
Emergency and Urgent Care
If you require emergency or urgent care when outside of the network, seek care at the nearest medical facility. Contact your primary care provider if assistance is needed to determine the appropriate level of care. Follow-up care must be received from an in-network provider unless approved by Quartz.
Mental, Behavioral Health and Substance Abuse
For assistance in accessing mental, behavioral health or substance abuse services, call Behavioral Health Care Management at 1-800-683-2300.
Contact your primary care provider for after-hour or out-of-area care instructions.
Quartz will continue to offer UW Health Care Anywhere, providing convenient access to the care you need from the comfort of your home, work or on-the-go. UW Health Care Anywhere video visits are available 24/7, using your smartphone, tablet or desktop computer webcam. Care Anywhere is available for members with common non-emergency symptoms.
