Uniform Dental
Uniform Dental is only available if you enroll in health insurance under the State of Wisconsin or Wisconsin Public Employers Group Health Insurance Program. It offers coverage for diagnostic, preventive, basic and children’s orthodontic services.
Your dental coverage will mirror your health insurance; if you elect family health insurance with dental, you will be enrolled in family dental coverage. If you elect individual health insurance with dental coverage, you will be enrolled in individual dental coverage. Uniform Dental is added to your health insurance premium.
Enrollment continues each year unless you cancel during the annual open enrollment period.
Preventive Plan
The new supplemental Preventive Plan mirrors Uniform Dental and is available to members who do not have health insurance under the State of Wisconsin or Wisconsin Public Employers Group Health Insurance Program. This Preventive Plan offers dental coverage for diagnostic, basic and children's orthodontic services. Members can purchase the Preventive Plan with or without the other Supplemental Dental plans.
Supplemental Dental
You can enhance your Uniform Dental Benefit or Preventive Plan coverage with:
- Delta Dental PPO™ - Select Plan
- Delta Dental PPO Plus Premier™ - Select Plus Plan
You can enroll in a supplemental dental plan without enrolling in Uniform Dental. You may only enroll in either the Select Plan or Select Plus Plan, not both.

Important to Note
- Employers do not contribute to the premiums for the Preventive, Select and Select Plus plans.
- Throughout the year, eligible employees may enroll or make changes to their enrollment based on qualifying events such as marriage, divorce, birth, adoption or loss of other comparable coverage. Go to ETF's Life Events Guide for a full list.
- With few exceptions, once enrolled, you must remain in the plan for the full year. Mid-year cancellation procedures follow those for health insurance—see the Frequently Asked Question “How do I cancel coverage.”
- You may cancel your new or existing enrollment during the annual open enrollment period.
- UW System and UW Hospital and Clinics employees may have different optional plans available. Check with your human resources/benefits office for details and instructions for 2020.
