The HealthPartners mobile app, MyHP, includes claim detail, easy touch screen log on and you can save your provider information for easy access. Whether you’re at home or on the go, your myHealthPartners plan information is right at your fingertips. From your smartphone, you can:

What’s New for 2023
HealthPartners is now in Southeast Wisconsin! HealthPartners Southeast covers Kenosha, Milwaukee, Ozaukee, Racine, Sheboygan, Washington, and Waukesha counties.
Referrals, Authorizations, Out-of-Network
Referrals are not needed when receiving care from in-network providers. Prior authorization is required for certain services, procedures and for out-of-network providers before you receive care. You will receive written notification of the approval or denial for the prior authorization. If you are unsure if a service, test or procedure requires prior authorization or a referral, contact 855-542-6922 prior to the appointment or visit Health Partner's State of WI for a complete list.
Emergency and Urgent Care
If you require emergency or urgent care when outside of the network, seek care at the nearest medical facility. If you are admitted to the hospital, you can contact HealthPartners at 800-316-9807 for help with coordinating your care and claim submission. Follow-up care must be received from an in-network provider unless otherwise approved by HealthPartners.
Mental, Behavioral Health and Substance Abuse
No referrals are necessary to see in-network behavioral health providers. If you need help finding a Behavioral Health provider, call our Behavioral Health Navigators at 888-638-8787.
For questions about whether or where you should see a doctor, home remedies or a medicine you're taking, call our Careline at 800-551-0859.
Get help from a nurse whenever you need it - even at 3 a.m. For questions about your pregnancy, contractions you are having, or your new baby call our Baby Line at 800-845-9297.
Save time and money. IYC Health Plan members can receive virtuwell and Doctor on Demand Services for free. High Deductible Health Plan Members pay up to $59 per visit. Get care right from your smartphone, tablet or computer. HealthPartners offers two telehealth options:
