It’s a different kind of health care model where providers and coverage coordinate to better care for you.

Dean Health Plan has you covered with 32 hospitals and many conveniently-located primary care sites in 20 counties throughout southern Wisconsin. Take advantage of one of the largest provider networks in southern Wisconsin with:

  • 5,200+ practitioners
  • 200+ primary care clinic locations, with many owned by SSM Health Medical Group
  • 950+ specialty care clinics

What’s New for 2023

SSM Health’s new South Madison Campus Clinic building opened for patient care on February 21, 2022. We are honored and excited to bring this investment in the health care services available to patients in the heart of Madison, where we have cared for generations of patients. The 5-story, 180,000 square foot building includes Family Medicine, Pediatrics, Internal Medicine, Obstetrics/Gynecology, Pelvic Health, Diabetes Education and Nutrition, Cancer Care with Infusion Services, Breast Care, Palliative Care, Health Psychology, Infection Prevention, Wound Care, Orthopedics, Sports Medicine, Podiatry, Physical Therapy, Occupational Therapy, Speech Therapy, Audiology and ENT. These specialties are fully supported through on-site ancillary services including imaging, lab and pharmacy services.

Provider Directory

Go to Dean's Resources for State of Wisconsin members page to access a provider directory. Select “Online” or “Printable Provider Directory” under "Providers and Information." To request a printed directory be mailed to you, call 800-279-1301.

Referrals, Authorizations, Out-of-Network

Referrals are not needed when receiving care from in-network providers. Prior authorization is required for certain services, procedures and for out-of-network providers before you receive care. You will receive written notification of the approval or denial for the prior authorization. You may check the status of your request at DeanConnect. If you are unsure if a service, test or procedure requires prior authorization or a referral, contact 800-279-1301 prior to the appointment.

Emergency and Urgent Care

If you require emergency or urgent care when outside of the network, seek care at the nearest medical facility. Contact the Customer Care Center at 800-279-1301, if possible, when you receive emergency care from an out-of-network provider. If assistance is needed to determine the appropriate level of care outside of normal working hours, contact the 24-hour Nurse Hotline (available to Wisconsin residents only) by calling 800-576-8773. Follow-up care must be received by an in-network provider unless otherwise approved by Dean Health Insurance.

Most often primary care providers (PCP) should be your first choice for care both because they are familiar with your medical history and because a scheduled office visit is the most cost-effective method of care.

When a more immediate need arises, you do have other options. Visit Choosing the right care for assistance in making the right choice when it comes to your need for care, ensuring that you receive the right care in the right place at the right cost.

Mental, Behavioral Health and Substance Abuse

A prior authorization is required for the following mental health and substance use services:

  • Detox
  • Inpatient
  • Residential
  • Partial Hospitalization/Day Treatment
  • Intensive Outpatient
  • In-home therapy

For members with mental health and substance use disorders, Dean Health Plan's behavioral health and substance use case management provides an individualized approach. The goal is to help you manage your health and live your best life. Learn more by visiting Behavioral health.
