ETF Web Forms is an online tool that gives members convenient, 24/7 access to a form, which is submitted directly to ETF for processing. This page provides information that will help you use the tool.  You must use a desktop computer and a Chrome, Firefox or Edge browser to use ETF Web Forms.

MyWisconsin ID

MyWisconsin ID allows citizens to create a single account, tied to a personal email address, to log in to participating Wisconsin government websites. Migrating applications over to using MyWisconsin ID will happen over the next several years.

ETF Web Forms uses MyWisconsin ID. If you do not have a MyWisconsin ID, you may sign up for one by selecting Login on the ETF Web Forms page. You will need to provide a personal email address to set up this account. Your email will then be your user name.

Create your MyWisconsin ID using a personal email address. You cannot sign up with a wisconsin.gov or wi.gov email address.

Before Creating Your Account

  1. Create a password. This will make the account creation process easier. Your password must meet the following requirements:
    • At least 16 characters 
    • A lowercase letter 
    • An uppercase letter 
    • A number 
    • A symbol 
    • No parts of your username 
    • Does not include your first name 
    • Does not include your last name 
    • Cannot be any of your last 24 passwords 
    • At least 2 hours must have elapsed since you last changed your password 
  2. Find your ETF Member ID. After creating your account, you will need to verify your identity with the following information:
    • Social Security Number or ETF Member ID
    • Date of Birth 
    • ZIP Code – Be sure to use the ZIP Code on file with your employer.  

Multi-Factor Authentication (MFA)

You will need to set up a security method to protect your account and ensure only you have access. There are four options for Multi-Factor Authentication, you only need to complete one. Most individuals are familiar with using their phone to authenticate. If you choose to authenticate with Google Authenticator or Okta Verify, download the application before creating your account.
