Group Health Cooperative of South Central Wisconsin (GHC-SCW) is a non-profit, member-owned health plan providing high-quality health care services, including primary care and specialty care, to over 70,000 members living in south-central Wisconsin.
Our Cooperative was founded on the revolutionary idea that patients should be in the driver’s seat of their healthcare system. They organized our Cooperative so that the member would be at the top of the leadership chart. This gives them a voice in their health care. Plan Options
You will enroll in one of two plans, depending on where you want to primarily receive care: the GHC-SCW Dane Choice Plan or the GHC-SCW Neighbors Plan. Regardless of where you live or work, you will have the option to select either plan.
The GHC-SCW Dane Choice Plan includes the following major health systems: Group Health Cooperative of South Central Wisconsin, UW Health, UnityPoint Health - Meriter and Access Community Health Centers.
If you primarily receive care at one of the healthcare systems listed above in Dane County, select the GHC-SCW Dane Choice Plan.
You will still be able to visit Urgent Care Facilities and Emergency Rooms outside of Dane County.
Note: All dependents on the subscriber's plan will automatically be enrolled within the same plan the subscriber selects. Subscribers and their dependents cannot be enrolled in different plans.
What's New for 2026
Olin Clinic Opening
The new Behavioral Health clinic on Olin Avenue opened on April 8, 2025. This location replaced the previous clinic on Regent Street, which permanently closed on March 14, 2025. This larger building allows for more psychiatry and therapy providers, along with space for our expanding Autism Spectrum Disorder program, and convenient parking and access for our members. Improved Online Provider Search Tool
An improved online provider search tool was launched in 2025. Members can now search for in-network providers by visiting our website, clicking on “Find a Provider,” choosing the applicable category (e.g. Provider, Behavioral Health Provider, and Clinic) and then selecting their network or product or “just shopping.” The GHC-SCW provider search tool will return a customized list of in-network providers with the ability to filter based on the member’s needs. User-friendly Strep Testing
GHC-SCW is excited to introduce a new and more user-friendly test for diagnosing strep throat. The test result still comes back in less than an hour, but because this new test is very accurate at diagnosing strep throat infections, you no longer need to wait up to two days for a second test (strep culture)! Patient After-Visit Surveys: Opting in for Text Message Notifications
Members can now receive text message notifications about their patient after-visit surveys that are available. Completing these surveys is an easy and convenient way to share your feedback with GHC-SCW. By submitting your survey, you help us better understand your experience and improve the quality of care we provide. Your insights allow us to make informed changes that enhance care, streamline processes and ensure that we continue to meet your needs. Learn more here.
New Vendor for GHCMyChartSM Video Visit Interpreters
GHC-SCW introduced a new vendor called Propio for providers to invite an interpreter, if needed, to your GHCMyChartSM Video Visit. This includes interpreters for 13 different languages.
Behavioral Health Fast Pass
Fast Pass is a convenient function for patients who utilize GHCMyChartSM and are on the behavioral health wait-list. Patients on the wait-list may receive text or email notifications that offer them a sooner appointment. Upon receiving this message, patients can log in to GHCMyChartSM and either claim the offer, if it is still available, or decline the offer and wait for another. If the patient does not respond to the Fast Pass offer, the offer will be considered declined.
PEERS® Social Skills Class
The Autism Spectrum Services Team at GHC-SCW expanded services for youth with Autism Spectrum Disorder (ASD). We now offer PEERS®, an in-person group therapy program for children with ASD ages 8 to 16 and their parents/caregivers. This is an evidence-based class that supports children to make and keep friends. Caregiver participation also supports children to learn the strategies they are being taught in this class. Updating Race and Ethnicity on GHCMyChartSM Profile
At GHC-SCW, we're dedicated to closing racial and ethnic health care disparities to ensure everyone gets the care they need. To help us provide the best care, members can now update their race and ethnicity information in their GHCMyChartSM profile. This information lets providers understand unique health needs and offer care that's right for their patients. It's about ensuring personalized care for everyone.
Organ Donation Registration in GHCMyChartSM
GHC-SCW is helping save and heal lives by being the first health care organization in the state to offer organ donation registration directly within its patient portal, GHCMyChartSM. This new feature makes it easier than ever for members to register as organ donors – a decision that could save lives.
The feature was developed in collaboration with Donate Life America and Epic, GHC-SCW’s electronic health record provider. It allows members to securely register with the National Donate Life Registry while managing their health online and reflects a shared commitment to removing barriers to life-saving decisions. Provider Directory
Go to our website and select “For Members.” Under “Information For” choose the “State of Wisconsin and Public Employer Group Members” option. Scroll down to the section “2026 Open Enrollment Directories and Maps” and select “2026 GHC-SCW Dane Choice Provider Directory.” To request a printed directory to be mailed to you, call Member Services at 800-605-4327.
Referrals, Authorizations, Out-of-Network
Your primary care provider will submit a referral request to GHC-SCW Care Management Department when you need to receive services outside of a GHC-SCW clinic or through a specialty care area that requires a prior authorization. Certain procedures or tests also require a prior authorization for medical necessity review. You will receive a letter from GHC-SCW, as well as notification in your GHCMyChartSM account, letting you know if the referral/prior authorization has been approved.
Emergency and Urgent Care
If you require emergency or urgent care when outside of the plan, seek care at the nearest medical facility. Contact GHC-SCW Member Services at 800-605-4327 if assistance is needed to determine the appropriate level of care. Follow-up care must be received from an in-network provider unless otherwise approved by GHC-SCW.

Virtual Visits
GHCMyChartSM Video Visits
Looking for a video visit? GHC-SCW members can schedule video appointments through GHCMyChartSM for routine visits and select specialty visits (PT/OT, Dermatology, and Behavioral Health). Video visit appointments through an active GHCMyChartSM are a simple option to see your GHC-SCW Care Team, while still getting the exceptional care you expect from an in-person visit.
Virtual Urgent Care, E-Visits, and Virtual Therapy Visits
Members can virtually access a doctor or therapist from their own home or office, on-the-go, 24/7/365. Licensed providers and therapists are available either by phone, secure video, or messaging to help treat any non-emergency medical or mental health condition. Visits are free for most members. Plan conditions may apply for members with HSA benefit plans.
