The insurance benefits decision guide is specially designed to help members with their health care decision-making process, as well as direct them to the collection of open enrollment web pages on the ETF website. As a reminder, employers must retain documentation that all employees either received email notifications or paper copies of the guide each year.
Insurance Benefits Decision Guide Distribution Options
We encourage you to direct your employees to the insurance benefits decision guide on the ETF website where a downloadable, printable PDF of the guide is available anytime. A future employer news item will outline the email notification process and provide a customizable electronic distribution message that you can share with your employees to provide them with a direct link to the online materials.
If you prefer to provide some or all of your employees with a paper copy of the guide, we will automatically send you the same number of guides as you received last year. If you would like to change the number of copies, contact name, or mailing address, email ETF at the address provided below with the information specified by June 18, 2025.
Action Required?
- If we don’t hear from you, ETF will provide you with the same number of paper guides you received last year. We will send them to the same delivery address and contact person. View your 2025 order of paper guides here.
- If you plan to send an email notification to all of your employees with a link to the guide on the ETF website, but have previously ordered paper copies, notify ETF that you no longer need any paper copies at ETFSMBEmployerInsurance@etf.wi.gov by June 18, 2025.
- If you would like to change the number of guides that you receive, or need to update the recipient or mailing address, notify ETF via email at ETFSMBEmployerInsurance@etf.wi.gov by June 18, 2025.
In your email, on or before June 18, 2025, please include:
- Employer name
- Employer number: 69-036- _______
- Name of contact person providing information
- Telephone number
- Delivery address (if different than the mailing address on file)
- Full email notification (Yes or No)
- Number of paper insurance benefits decision guides requested
