What's New for 2026
Enrollment, ID cards, and member tools
- New ID cards: Cards with new member IDs and group numbers will be issued. All current medical authorizations that exist will carry over to your new member ID. Please be sure to present your new ID card to your providers for claim submissions.
- Member Portal: The member portal will have a new look and feel but your credentials remain the same.
- If you have not created a portal account, please create an account once you receive your new ID cards to view your benefit information, claims information and more.
- Primary Care Provider (PCP)/Primary Care Clinic (PCC) changes will need to go through Member Services.

Mayo Clinic Health System Opens New Orthopedics and Sports Medicine Clinic (Mankato)
The new clinic offers comprehensive sports medicine care to athletes and active adults in Mankato and the surrounding area, featuring a team of expert orthopedic surgeons and athletic trainers, as well as advanced digital tools for convenient patient access to care. The clinic's location at The Pond on Madison – a regional premier hockey facilitate - further enhances its connection to the local athletic community.

Referrals, Authorizations, Out-of-Network
Referrals are not needed when receiving care from in-network providers.
Prior authorizations are required for certain services, procedures, and for out-of-network providers before you receive care. A prior authorization is a written request that is completed by a network Primary Care Provider and/or network specialist provider. It requests authorization for a specific service(s) or procedure to be performed by an in-network provider or, in some cases, an out-of-network provider.
Prior authorization is required for care from any out-of-network providers unless it is an emergency or urgent care.
Access the PATIENT PORTAL to help you manage your care and give you secure and convenient access to all your health information. To create a Mayo Clinic patient portal account go to the patient portal page or download the Mayo Clinic App from Google Play or the App Store.

Emergency and Urgent Care
If you require emergency or urgent care when outside of the network, seek care at the nearest medical facility. Contact Member Services at 877-371-6762, if possible, when you receive emergency care from an out-of-network provider. Follow-up care must be received by an in-network provider unless otherwise approved by Dean Health Plan.
Most often, Primary Care Providers should be your first choice for care. This is because they’re familiar with your medical history and a scheduled office visit is the most cost-effective method of care.
Primary Care On Demand is a mobile app that offers virtual care 24/7 for you and your family members ages 3 and older. Access care when and where you need it. To learn more, search Primary Care On Demand or download the app on the App Store or Google Play.

Virtual Visits
EXPRESS CARE VIRTUAL ONLINE – Express Care offers quick, convenient virtual care and video visits for common conditions to patients ages 18 months through 75 years, unless otherwise noted. Learn more.
PRIMARY CARE ON DEMAND – This app-based service is available 24/7/365 for quick virtual care and on-demand video visits with physicians. Get quick care for patients ages 3 and older in under 30 minutes.
VIDEO APPOINTMENTS - Video appointments are another way Mayo Clinic Health System provides you care. You can start one from home on a mobile device or computer by using your Patient Online Services account. These videos show you how to log in to your video appointments from a computer and a mobile device.
