When You Are a New Hire
Enrollments must be completed within 30 days of your date of hire in an eligible position.

When You Experience a Life Change Event
To enroll or make changes to your benefits due to a qualifying life event, complete a Health Insurance Application/Change Form (ET-2301) within the designated timeframe. Return the form by submitting it to the UW Health HR Service Center through an HR Service Request.
