Document Requirements for Life Events and Dependent Verification

Find out which documents are required when adding a spouse or dependent to your insurance or making changes due to a qualified life event. Having the correct paperwork will help ensure a smooth update to your coverage.

Make Changes

Refer to the life event pages above for detailed instructions on how to make changes.

Retirees: You may cancel health and Uniform Dental coverage or change from family to individual coverage at any time. To make changes online: 

  • Log in to My Benefits to edit your benefits.
  • Select "Request to cancel or reduce coverage " as your reason for change.
  • Enter the last day you wish to be covered and update your benefits.

Alternatively, you can submit a written and signed cancellation request to ETF.

Supplemental benefits can only be canceled due to a qualifying life event or during open enrollment.
