Complete the survey to request an on-site flu clinic. Submit your request early to ensure your preferred date(s) can be accommodated. Requests will be confirmed by email within 2 weeks of submission.

Note: UW-Madison should disregard this information since they use a different administrator to coordinate flu vaccine clinics.

The Daily Habits program available through Well Wisconsin helps employees work toward their well-being goals with daily reminders, helpful tips, and goal-tracking features. A variety of plans are available including, but not limited to: Enjoy Exercise, Lose Weight, Keep Stress in Check, and Sleep Well. Participation also counts toward the $150 incentive.
