Summary

If you wish to specify who shall receive a primary beneficiary’s share of a death or life insurance benefit if a primary beneficiary is deceased, you must use this alternate beneficiary designation form.

WRS death benefits are always paid according to the most recent, valid beneficiary designation on file with ETF prior to your death. If a beneficiary designation is not on file, WRS death benefits will be paid according to statutory standard sequence. Your beneficiary information does not automatically change when a life event occurs, such as a marriage or a divorce.

Note: The date the form is signed is not the date it becomes effective. A Beneficiary Designation form does not become effective until received and approved by ETF. The person filing the designation must still be alive when ETF receives the form. An acknowledgment will be sent when this designation has been reviewed and accepted. Invalid designations will be rejected. ETF's acceptance of your designation does not guarantee that your account will provide a benefit upon your death. Contact ETF with questions about a potential death benefit from your account.
