This report presents the results of the annual actuarial valuation of benefit liabilities and costs of the Duty Disability Program as of December 31, 2019.
Each time a guardian makes a request on behalf of a member, ETF will review the Letters of Guardianship of the Estate. The request will be approved or denied based on the authority granted in the letters.

Your WRS account information is confidential, and no one can receive information about or make changes to your account unless authorized. You can authorize another person to make changes to your account by submitting a financial power of attorney document to ETF.
Employers should submit this form when first aware that an insured employee is unable to work due to illness or injury and will be unable to perform any work or to engage in any occupation for an indefinite period.
