The State of Wisconsin Group Health Insurance Program (GHIP) provides uniform health benefits to both state agencies and local government entities. Nearly a dozen health plans participate in the GHIP providing coverage to nearly one-quarter million lives.

The ASLCC and SHICC programs were created by Wis. Stat. § 40.05 (4) (b). The ASLCC Program was first authorized in 1972; the SHICC program was first authorized in 1995.
New health plans should use this form to request that certain materials submitted to ETF with the New Health Plan Application be kept confidential under the Wisconsin Open Records Law.
