Use the Designation of Agent (ET-1313) form to add, change, or update your:

  • Agent: Responsible for understanding and administering all aspects of administering the WRS. Serve as the contact through which all WRS communications will be sent.
  • Alternate agent: Acts on the agent’s behalf if the agent is unavailable. Alternate agents are recommended but not required.
  • Retirement contact: Responsible for WRS eligibility determinations, reporting, etc. Serves as the primary contact for all WRS inquiries from ETF.
  • Insurance contact: Responsible for ETF-administered insurance program eligibility determinations, reporting, etc. Serves as the primary contact for insurance inquiries.
