Direct deposit is the most secure, reliable and convenient method of receiving benefit payments and is generally the only payment method used by ETF.
Follow these steps each time you change banks to update your direct deposit information:
- Print, complete, date and sign the Direct Deposit Authorization (ET-7282). The completed form can be faxed to 608-267-4549 or mailed to:

- Contact your bank if you have questions regarding the account information on the form.
- Leave your old account open until at least one deposit is received in your new account to avoid missing payments.
Acceptable Financial Institutions
ETF can electronically transfer your annuity payment to any financial institution participating in the National ACH system. This includes a financial institution account held in your name or an account owned by a representative payee, nursing home, religious order, or other designated entity. Accounts owned solely by trusts will not be accepted. ETF cannot transfer payments internationally.
If another person holds an interest in your account, identify them on the Direct Deposit Authorization (ET-7282). If any person who holds an interest in your account changes, notify ETF of the change in account ownership.
