If you need to correct or change your personal information or that of your dependents/named survivors, you may be asked to provide acceptable documents for verification. This is to maintain data integrity, provide the highest standard of service possible, and ensure the accuracy of our records.

How to Submit Documents

  • Active employees: Do not submit to ETF. Contact your employer. They must update ETF.
  • Retirees, inactive members, other benefit recipients: Submit to ETF by mail, fax, or drop off.

Find acceptable documents for changes to your:

Usually, no other documentation is required. However, if ETF determines additional requirements, we will accept a copy of any of the following:

  • REAL ID-compliant state-issued driver’s license, instructional permit, or identification card
  • Wisconsin DOT-issued driver’s license, instructional permit, or identification card
  • Birth certificate (certified copy)*
  • Unexpired U.S. passport
  • Hospital birth record
  • Naturalization (citizenship) certificate
