State Employee and Retiree Health Plan & Supplemental Benefits
A resolution for the employer to offer the Wisconsin Public Employers Group Health Insurance program to eligible personnel through the program of the State of Wisconsin Group Insurance Board, and agrees to abide by the terms of the program.
Survivor's certification that a WRS annuity or lump-sum payment made to the survivor of the member named in this form may be eligible for an income tax exemption.
Today, the Group Insurance Board set premium rates for the 2026 plan year. In addition, the Board approved the health plan service area qualifications and other benefits of the State of Wisconsin Group Health Insurance Program.
