The dental plans available to you are determined by whether you are an employee or retiree, and who is your current or former employer.

State Employees & Retirees

You are considered a state employee or retiree if you work for or have retired from any state agency (including STAR and non-STAR), University of Wisconsin, UW Hospital & Clinics, or legislature.

You are eligible for:
  • Uniform Dental if you are enrolled in health insurance through the Group Health Insurance Program
  • The Preventive Plan if you are not enrolled in health insurance through the program
  • The Select Plan
  • The Select Plus Plan

Please note you may only enroll in the Select or Select Plus Plan. You can't enroll in both.

Local Employees

You are considered a local employee if you work for a municipality such as a city, township, village, school district, or technical college.

You are eligible for Uniform Dental if:
  • Your employer offers health insurance through one of the following Group Health Insurance Program options:
    • Local Traditional Health Plan (PO2)
    • Local Deductible Health Plan (PO4)
    • Local Health Plan (PO6)
    • Local High Deductible Health Plan (PO7)
  • You are enrolled in health insurance through the program

You are eligible for the Preventive Plan if your employer offers it and one of the following statements is true:
  • Your employer does not offer Uniform Dental
  • You are not enrolled in health insurance through the Group Health Insurance Program
Your employer may also offer the Select and Select Plus plans

Local Retirees

You are considered a local retiree if you retired from a municipality such as a city, township, village, school district, or technical college.

You are eligible for Uniform Dental if:
  • Your former employer offers health insurance through one of the following Group Health Insurance Program options:
    • Local Traditional Health Plan (PO2)
    • Local Deductible Health Plan (PO4)
    • Local Health Plan (PO6)
    • Local High Deductible Health Plan (PO7)
    • Local Annuitant Health Plan (PO8)
  • You are enrolled in health insurance through the program

You are eligible for the Preventive, Select, and Select Plus plans
  • You may only enroll in the Select or Select Plus Plan. You can't enroll in both.
  • You can only enroll in the Preventive Plan if you aren't enrolled in Uniform Dental.
