Overview of Plan Options

Step 1: Sign up for basic coverage

Choose one plan to get covered for basic procedures such as cleanings, exams, fluoride treatment, fillings, and orthodontia.

Uniform DentalDelta Dental PPO Plus Premier™ - Preventive Plan

Uniform Dental is available to those enrolled in health insurance under the State of Wisconsin Group Health Insurance Program.

Uniform Dental coverage mirrors your health insurance coverage. Example: If you elect family health insurance with dental, you will be enrolled in family dental coverage.

Preventive plan is available to those not enrolled in Uniform Dental and it offers the same coverage as Uniform Dental.

Your coverage doesn’t need to mirror your health insurance coverage. You can choose from individual or family coverage.

Step 2: Add more coverage

Choose one plan to add more coverage for major dental services.

Select plan covers services like crowns, bridges, root canals, dentures, oral surgery, and implants.In addition to Select plan benefits, Select Plus has a lower deductible and higher benefit maximum, offers more dental providers, and covers orthodontia for children and adults.

You can elect individual, individual + spouse, individual + child(ren), or family coverage. Your coverage level doesn't need to mirror your health insurance coverage.
