You can enroll in supplemental benefits during the annual open enrollment period in the fall, within 30 days of your date of hire, or when you experience a qualifying life event.
Enrollment in supplemental benefits continues each year unless you cancel during the open enrollment period. You cannot cancel your benefit election(s) mid-year without a qualifying life event*. When making a change to your benefits due to a life event, review the Life Events Guide page for deadlines, coverage effective dates, and documentation requirements.
*Retirees may cancel Uniform Dental coverage at any time.

- Uniform Dental will match your health insurance effective date
- Vision, Accident Plan, and all other dental plans are effective on the first of the month on or following your hire date

Employees
How to enroll or make changes to your supplemental benefits depends on your employer.
State employees who work for agencies covered by STAR: Log in to STAR to enroll or make changes to your benefits during open enrollment or when you are a new employee. Contact your employer for directions on how to make changes due to a qualifying life event.
Universities of Wisconsin Employees: You may have different supplemental benefits available. Visit the UWs Employee Benefits page for a complete list of supplemental benefits and instructions on how to enroll or change benefits.
UW Hospital & Clinics Employees: You may have different supplemental benefits available. Visit The Pulse or contact the UW Health HR Service Center at 1-608-263-6500 for a complete list of supplemental benefit and instructions on how to enroll or change benefits.
All other employees: Contact your employer for directions on how to enroll, change, or cancel your benefits.
Employees About to Retire
Retirement does not provide an opportunity to enroll in the Accident Plan, vision insurance, or dental plans. To continue existing coverage, you will need to complete the appropriate paperwork. Talk with your employer benefits specialist for more information.
- Uniform Dental: Uniform Dental continues automatically if you continue your health insurance.
- Preventive, Select, or Select Plus dental plans: Complete the Supplemental Dental Enrollment Form and mail the completed form to Delta Dental at the address on the bottom of the form. This form is due within 30 days of your retirement date.
- Vision: Complete the Supplemental Vision Retiree/Continuant Change Form and mail the completed form to Delta Dental at the address on the bottom of the form. This form is due within 30 days of your retirement date.
- Accident Plan: You can continue coverage after you leave state employment or retire, up to age 70. Your plan will be converted into an individual, non-ETF plan. COBRA Continuation Coverage
If you continue your coverage at retirement through COBRA, your COBRA continuation coverage will expire after 18 months. To continue coverage, enroll as a retiree during open enrollment within those 18 months. If you do not enroll during this time, your coverage will end, and you must wait until the next open enrollment period to re-enroll.

Uniform Dental
To enroll or change your coverage level, submit a Health Insurance Application/Change For Retirees (ET-2331) form to ETF. Please note, your Uniform Dental coverage mirrors your health insurance coverage. Example: If you elect family health insurance with dental, you will be enrolled in family dental coverage. So in order to change Uniform Dental coverage, you must also change your health insurance coverage.
During open enrollment, you may login to myETF Benefits.
- myETF Benefits is only available for currently enrolled retirees
- myETF Benefits cannot be used if some people on your health insurance are enrolled in Medicare and some are not enrolled in Medicare
To cancel Uniform Dental, you must provide ETF with a written and signed letter stating you are voluntarily canceling coverage.
