After saving your sick leave credits, you can re-enroll in the Group Health Insurance Program during one of the following times:
- The annual open enrollment period, or
- Within 30 days of losing coverage, once you lose coverage of a comparable health insurance plan
During Open Enrollment
To re-enroll during open enrollment, you must submit the following to ETF:
- Group Health Insurance Application/Change for Retirees (ET-2331) form
- Sick Leave Re-Enrollment Application (ET-4317)
- Summary of Benefits and Coverage from your plan with comparable coverage
You can choose the first of any month in the following year for your health insurance coverage to start.
After Losing Comparable Coverage
You must re-enroll within 30 days of when your comparable coverage ends. This must be an involuntary loss of employer health insurance (see the Sick Leave Re-Enrollment Application (ET-4317) for more information). To re-enroll, you must submit the following to ETF:
- Sick Leave Re-Enrollment Application (ET-4317)
- Summary of Benefits and Coverage from comparable coverage
- Loss of coverage letter (this letter should include the name of the organization you had coverage under, name of the insurance group, date coverage terminated, and the reason eligibility for coverage was terminated.)
- Group Health Insurance Application/Change for Retirees (ET-2331)
